Sir, ? Very young children have difficultiesin swallowing capsules due to their age,mechanical ventilation, and/or sedoanalgesiaimmediately post liver transplantation. Inthese specific situations, administrations ofsyrup or intravenous (IV) formulations arerarely used because of their instability, costs,and adverse events [1]. Thus, sublingual (SL)administration of tacrolimus is an interestingalternative in these cases because of its easeof administration besides its adequate exposureachieved [2]. Limited information of tacrolimusperformance after SL administrationis available [3, 4], and specifically there areno reports in pediatric patients. Therefore, weaimed to describe the dosage, exposure, drugdruginteractions, clinical, and safety parametersof tacrolimus SL administration in pediatricliver transplant patients in a retrospectiveobservational study (Project #889).
