AbstractAdvances in tobacco control in Brazil can bereflected in the decrease in prevalence overthe past two decades. Death statistics and theoccurrence of events and direct costs attributableto tobacco-related diseases have not beenfrequently estimated in the country. The goal ofthis article is to estimate the burden of smokingin 2011 regarding mortality, morbidity andmedical care costs of the main tobacco-relateddiseases. A probabilistic microsimulationhealth economic model was built. The modelincorporates the natural history, costs andquality of life of all the tobacco-related adultspecificdiseases. Smoking was accountablefor 147,072 deaths, 2.69 million years of lifelost, 157,126 acute myocardial infarctions,75,663 strokes, and 63,753 cancer diagnoses.The direct cost for the health system was ofBRL 23.37 billion. The monitoring of tobaccorelatedburden is an important strategy to guidedecision-makers and to strenghten health publicpolicies.Cost of Illness; Smoking; Potential Years of LifeLost; Mortality
