Thoracic involvement at diagnosis drives a differentiated clinical presentation of sarcoidosis: analysis of 1245 patients from the SarcoGEAS-SEMI Registry

Resumen:
OBJECTIVE. To analyse whether thoracic involvement at diagnosis is associated with a specific clinical presentation of the disease in a large nationwide registry of patients with sarcoidosis. Methods. In January 2016, the Autoimmune Diseases Study Group (GEAS-SEMI) created a national registry (SARCOGEAS) of patients with sarcoidosis. Sarcoidosis was diagnosed in agreement with the criteria proposed by the American Thoracic Society/European Respiratory Society/World Association of Sarcoidosis and Other Granulomatous Disorders (WASOG) 1999 statement on sarcoidosis. Organ involvement was retrospectively determined in each patient at the time of diagnosis using the 2014 WASOG organ assessment instrument. Radiographic stages at diagnosis were classified as stage 0 (normal radiological study), stage I (BHL without pulmonary infiltrates), stage II (BHL plus pulmonary infiltrates), stage III (parenchymal infiltrates without BHL) and stage IV (extensive fibrosis with distortion or bullae).Results. The cohort consisted of 1245 patients (81% biopsy-proven), including 722 (58%) women and 523 (42%) men, with a mean age at diagnosis of 47.2 ± 15.3 years. With respect to the FDA ethnic classification, 1138 (91%) patients were classified as White, 57 (5%) as Hispanic, 18 (1%) as Asian and 32 (3%) as others. Pulmonary imaging data at diagnosis was available in 1230 patients: thoracic involvement was detected in 1132 (90%) patients, including 395 (32%) patients presenting with stage I, 500 (40%) with stage II, 195 (16%) with stage III and 42 (3%) presenting with stage IV. Patients with no thoracic involvement at diagnosis (stage 0) were more frequently women (73% vs 56%, p=0.002), had a mean older age (52.1 vs 46.8, p=0.001) and had a higher frequency of skin (54% vs 34%, p
