Background: Cardiovascular disease (CVD) risk is very high in chronic renal failure(CRF), but the underlying mechanisms are not understood. Traditional cardiovascularrisk factors (RF) do not explain the increased risk.Objective: We investigated the relation of impaired renal function to carotid total plaquearea (TPA) in healthy controls, and in stage 2 and 3 CRF.Methods: Participants consented to a protocol approved by the Blossom DMOArgentina ethics committee. Clinical and laboratory tests and TPA were determined attime 0 and after 1 year. TPA was measured using carotid ultrasonography. Renalfunction was determined by MDRD equation 4. Results: NRF (age 46±2 yrs.) had eGFR84±6 ml/min, blood pressure (BP) 135±3/82±2 mmHg, BMI 31±1, LDL113±11 mg/dl,HbA1c 6.7±0.4% and had the lowest TPA 31.8±6.3 mm2. Stage 2 CRF (52±1 yrs.) hadeGFR 70±1 ml/min, BP 132±1/81±1 mmHg, LDL 131±11 mg/dl, HbA1c 6.2±0.1% andTPA 53±6mm2, while Stage 3 CRF (59±1 yrs.) had eGFR 56±1 ml/min, BP139±2/83±1, LDL130±3 mg/dl, HbA1c 6.2±0.1% and TPA 72±10mm2 (p
