Snomed CT and ICPC-2E cross mapping: Coding to facilitate the access to information

The representation of medical diagnosis using a controlled vocabulary is an important task in many areas as epidemiological analysis of populations. During four years 521,971 problems were generated in ambulatory encounters using our problem oriented electronic medical record (ITALICA). These texts were coded with 5,760 SNOMED GT concepts in 7,108 different combinations with a high level of detail. However, for certain purposes related to legacy systems we continued to use ICPC-2 for the coding of the problem list mapping this classification against SNOMED GT without using the ICD-10 or 9 cross mapping SNOMED GT reference table. In our experience 6.961 relations were created between SNOMED GT and ICPC-2 that affected 463.184 medical problems. We also found 58.787 texts that did not agree with our definition of "medical problem" although 99% of them could be represented using SNOMED Cl If a problem was coded with SNOMED GT it was always possible to be coded with ICPC-2, although sometimes in a "rag bag" category. There was a small group of 24 texts that agreed with our definition of "medical problem" that could not be represented in any of the two classifications. Topic: Technology & Medicine
