Documentación de alergias medicamentosas: implicancias actuales y futuros desafíos

Improper documentation of drug allergies cause a significant rate of hospitalization. Objective: To analyze the quality of the documentation of drug allergies at 750-bed university hospital. Method: A retrospective cohort of patients with at least one severe allergy symptom between January 1998 and May 2002. Results: We reviewed the electronic medical records of 608 patients. We found 345 (56.74%) patients with drug allergy. Only 8.69% had the drug coded, the rest had the causal medication information in the free text of the clinical notes. Conclusion: Multidisciplinary strategies must be implemented to improve documentation of allergic reactions and especially the medication that triggered the effect to prevent this kind of adverse drugs reactions.
