POST-TRASPLANT LYMPHOPROLIFERATIVE DISORDERS IN PATIENTS RECEIVING SOLID ORGANS. CASE SERIES AND INCIDENCE OF EPSTEIN-BARR VIRUS

Post-transplant lymphoproliferative disorders (PTLD) comprises a heterogeneousgroup of lymphoid proliferations that present after a transplant in 1-2% oforgan recipients.The objective of this research is to describe the cases of PTLD diagnosedin our institution (Hospital Italiano de Buenos Aires) between 2002 and 2017,in terms of morphological, immunophenotypic characterization according to theWHO criteria of 2016, and to study the tissue prevalence of Epstein-Barr Virus(EBV).Data of all cases diagnosed as PTLD in our hospital was obtained fromthe institutional data center. Histological material was obtained from archive.Immunohistochemical techniques were performed to reclassify the cases accordingto the WHO criteria (2016). In situ hybridization was performed to study thepresence of EBV in the tissue.The study population consisted of 40 cases, of whom 20 werefemale and 20 were male. 55% of the patients were pediatric and 45% wereadults. The age range was 1 to 73 years (mean 28.48 +/- 24.42 years). 52.5% ofthe transplants were hepatic and 47.5% were renal. The localization of thetumors was heterogeneous, mainly involving lymph node (37.5%). Regarding to the histological pattern, 2.5% correspondedto Non-Destructive PTLD, 60% were Monomorphic PTLD, 27.5% were Polymorphic PTLDand 10% were Hodgkin-like PTLD. The prevalence of EBV was 62.5% (25 positivecases).In conclusion, the data in our series is coincident with data reported ininternational series regarding morphological classification and EBV status. Thepresentation of this series of cases is the starting point of a largerinvestigation, to study prognostic and predictive factors, to achieve arisk-stratification.
