Epilepsy is the most common serious chronic neurological disorder, and affects more than 50 million people around the world. It is characterized by recurrent seizures, both local and generalized, and although in the last decade more than ten new antiepileptic drugs (AEDs) have been developed, about a third of patients remains resistant to AEDs therapy: approximately 20% of the patients with primary generalized epilepsy and up to 60% of the patients with focal epilepsy develop drug resistance. One of the most studied hypothesis is the multidrug transporter hypothesis, where high expression levels of polyspecific efflux pumps of the ABC (ATP-binding cassette) superfamily, such as P-gp or BCRP, restrict substrates transference through the blood brain barrier (BBB) and/or epileptogenic focus, which generates the need of high doses administration of AED, causing severe adverse effects. In order not to affect the physiological or basal function of this transporters, one strategy has become more relevant: the encapsulation of AED into solid lipid nanoparticles (SLN). These nanoparticles are characterized by a higher permeation through BBB and retention in brain blood capillaries. Our primary aim is to develop, prepare and evaluate new therapeutic systems for the refractory epilepsy treatment, nanoscale and biopharmaceutically optimized to enhance cerebral bioavailability of administered drugs.
