One of the main challenges in automatic glycemic regulation in patients with type 1 diabetes (T1D) is to dispense with carbohydrates counting. In this context, an automatic meal detection algorithm based on a Kalman filter and continuous glucose monitoring (CGM) readings is proposed to be integrated to the previously introduced Automatic Regulation of Glucose (ARG) algorithm. This closed-loop strategy not only regulates the basal insulin infusion rate but also generates insulin spikes at meal times, i.e., no open-loop insulin boluses are needed to mitigate postprandial excursions. However, it is still required to announce the meal time and classify the meal size between three categories: small, medium or large. In this work, the performance of the proposed meal detector integrated into the ARG algorithm is evaluated in silico and with clinical data obtained from the first-ever artificial pancreas (AP) trials carried out in Latin America.
