COMPARISON OF THE EFFECT OF THE ORALLY AND TRNSDERMALLY ADMINISTRATED HORMONAL REPLACEMENT THERAPY (HRT) ON THE LIPID AND LIPOPROTEIN PROFILE.

Resumen:
To evaluate the oral and transdermal administration of cyclic HRT, a randomized tranversal study was designed. The studied group was made up of postmenopausal women (PMW), n=35, under three different therapies: a) Oral administration of Equine Conjugated Estrogens (ECE) 0.625 mg/day, n=10; b) Trandermal administration of 50 µg 17b-Estradiol by a Reservory Trandermal System (RTS) two days a week n=10; c) trandermal administration of 50 µg 17b-Estradiol by a Matrix Transdermal system (MTS) two days a week n=10. every treatment was administrated for six months and was combined with a cyclic, sequential dose of 5 mg/day Medroxyprogesterona Acetate (MPA). Total, VLDL, IDL, LDL and HDL Cholesterol, and total triglycerides in sera were determined pre- and post-treatment. In group a) a significant decrease in Total and LDL cholesterol was observed (240±17 vs 223±11, p< 0.001 and 164±15 vs 143±9.5, p<0.05, respectively), with no changes in VLDL, IDL, HDL Cholesterol and total triglycerides. In group b) and c) there was no difference in the parameters evaluated, except for a decrease in HDL Cholesterol in group b) (57±3.2 vs 52±3.4, p<0.05) even though the values were within the normal ranges (>35 mg/dl). Following six-month treatment, oral administration of HRT was more efficient than trandermal administration given the decrease observed in total and LDL cholesterol, with no changes in HDL cholesterol.
