COMPARATIVE STUDY OF TWO TWICE-A-WEEK TRANDERMAL ESTRADIOL DELIVERY DEVICES.

Resumen:
We have studied 15 healthy post menopausal women with amenorrhea > 1 year and FSH > 40 mUI/ml, in order to compare the effects on endometrial cytology, lipid profile and SHBG of two twice-a-week trandermal estradiol (E2) delivery devices: a reservoir trandermal system (RTS) and a matrix trandermal patch (MTS), both delivering 50µg/day of estradiol, in a continuous regimen, during a 180-day period. The patients were divided: Group 1(n=10) treated with RTS, and Group 2 (n=15) with MTS. Both received 5 mg/day of medroxyprogesterone acetate (MPA) 14 days/month. At time 0, 3 and 6 months they underwent a clinical examination. Endometrial and blood sampling [SHBG, total cholesterol (Ch), HDL-Ch (HDL), LDL-Ch (LDL), IDL-Ch (IDL) and triglycerides (TG)] was at time 0 and 6 months. Both treatmentws alleviated climacteric symptoms showing no cytological changes. Results (mean±standard error): SHBG 108.3±7.8 vs 85.3±9.6 (p<0.0005), all values within normal range, producend by E2 delivered by the patches. Atherogenic lipoproteins had no significant change except a small decrease in HDL in Group 1 (57.0±3.0 vs 52.0±3.0 mg/dl, p<0.05) within expected values, probably due to less bioavailable ·E2 from RTS patches, unable to neutralize the androgenic effect of MPA, but sufficient to stimulate hepatic synthesis of SHBG.
