Factors associated with inhospital mortality in HIV positive patients in Argentina

Background: To explore risk factors associated with in hospital mortality of HIV+ patients in a public hospital in Argentina. Methods: Retrospective observational study. Data from HIV+ adult, non-pregnant patients admitted between 01/2004-12/2007 was obtained from clinical records. Items searched: age, gender, time from HIV diagnosis, previous opportunistic infections (OI), CDC clinical stage, CD4 count and viral load (VL) at admission and diagnosis. Primary outcome measured was in-hospital mortality. SPSS software performed statistical analyses. Results: 712 admissions (546 patients) were recorded. Median age: 38 years (range: 20-73). Male: 73.5%. Heterosexual 39.9%, MSM 23.4%, IDU 28%. Median time from HIV diagnosis: 5 years (range: 0-21). HIV was diagnosed during hospitalization in 22.6% of the patients. At admission: median CD4 cell count, 65/ mm3 (range 0-1183); median VL 60,171 copies/mm3 (range 50-750,000). Category C at entry: 88%, out of them 57% had previous opportunistic infections. 35.6%% were on HAART. 78% (n: 555) of admissions were HIV related. 141 patients (25.8%) died while confined, mainly associated with opportunistic infections.
