Analysis of the causes that gave origin to a leak that took place close to PK 56+436 of Camisea NGL Pipeline, operated by Coga. Developed activities include the generation of a cause and effect tree and the definition of immediate and Root Causes. No material non-conformities, weld defects or other contributors due to construction or operation deficiencies were found. The buckle was due to soil movement, mostly rotational, that created a displacement mostly longitudinal to the NGL pipeline. The soil movement was probably generated during the rainy season a year before the failure took place, and kept growing during last season, as a consequence of heavy rains in an unstable surface soil layer. The most likely moment for the generation of the main part of the land-slide that caused the failure was during the rainy season 2010-2011.
