Mother to child transmission (MTCT) is the only transmission route for HIV which can be reduced with intervention strategies by the health team. The goal of this work was to follow-up the dynamics of HIV infection in children consulting at the Children?s Hospital Sor María Ludovica, La Plata, Argentina. Description: A prospective longitudinal study was performed in 1.371 HIV exposed children attending at the Infectious Diseases Department, Children?s Hospital Sor María Ludovica, Buenos Aires Province, from 01/03/1987 to 31/12/2005. In order to perform diagnosis of infection for HIV the Criteria from CDC, and the Argentine Recommendations were followed. Studies of p24 antigen and PCR were performed at the National and Provincial Reference Laboratories. The global HIV transmission rate was 28.2 % and by year it was reduced from: 75.0 % and 83.3 % in 1988 and 1989, respectively; to 19.0 % and 21.6% in 2004 and 2005, respectively. Medical intervention (protocol 076 or HAART +/- cesarea) was performed with a growing tendency from 24.6 % in 2000 to 45.4 % in 2004.
