Study of Delayed Ventricular Activation After Myocardial Infarction

Several studies haveshown that after myocardial infarction (MI), patients have a high incidence ofventricular arrhythmias. In this work, we have evaluated the delayed ventricularactivation during healing (within the first 7 days after MI; MI7) and healed(60 days after MI; MI60) stages. We carried out a comparative study between theQRS spectral variance (QRSSV ) and the Signal-averaged Electrocardiogram(SaECG). The electrocardiogram (ECG) of 49 healthy subjects, ECG of 84patients in MI7 and ECG of 41 patients in MI60 were analyzed utilizing theFrank XYZ leads. Results indicate that QRSSV showed a significantincrease (p<0.05) in Frank XYZ leads during MI7 with respect tocontrol. Also in MI60, the QRSSV in VY lead tends to decrease (p<0.05)respect to MI7. Moreover, we have observed an increment of SaECG in MI7 anda tendency to decrease in MI60 with respect to control, without statisticalsignificant differences between groups. This study might help understand theinfluence of the time elapsed after MI on modulation of QRSSV . Also, QRSSVcould be detecting disorder relate to delay activity which is unstable,while SaECG could be restricted to locate of delayed activity which isstable along the time.
