Abstract Introduction.Recent reports have shown several cases of cerebrovascular events aft er vaccinati on againstCOVID-19. The eff ects have been described mainly in women within the fi rst two weeks of receiving thevaccine.Clinical Case.We describe here the fi rst Colombian case of a cerebrovascular event aft er vaccinati onagainst COVID-19 in a 67-year-old woman with a vascular history. Four days aft er applicati on of themessenger ribonucleic acid (mRNA) vaccine, she exhibited deviati on of the labial commissure, ipsilateralptosis, and limitati on of march with lateralizati on. The event was associated with a subacute ischemicevent in the right thalamus in parasagitt al situati on, changes in chronic ischemic microangiopathy of smallvessels, and vascular crossing in the right cerebellar angle, without other alternati ve causes.Conclusion.The development and rapid use of vaccines has allowed the hospitalizati on and mortalitystati sti cs associated with COVID-19 to be reduced, but at the same ti me, it has generated concern aboutthe potenti al side eff ects, generati ng controversy among the general populati on, especially in individualswith cardiovascular diseases. In our case, we provided evidence for the discussion of potenti alcerebrovascular events related to the applicati on of vaccines in older people with a history ofcerebrovascular diseases. This was done in order to analyze and control in subsequent studies themodulati on of medical history on the likely eff ects of vaccinati on. However, despite the unavoidable sideeff ects, the benefi ts of vaccinati on are superior.Keywords: Ischemic stoke, COVID-19, vaccinated
