REMODELAMIENTO OSEO DURANTE EL EMBARAZO:
EFECTO DE LA SUPLEMENTACION CON DOS FUENTES DISTINTAS DE CALCIO
EN MUJERES CON INGESTAS DE CALCIO HABITUALMENTE BAJAS

RESUMEN
Este trabajo longitudinal estudia cambios en el remodelamiento ¨®seo durante el embarazo,
en 150 mujeres sanas del Hospital D. Paroissien, La Matanza (Secci¨®n Obstetricia) con ingestas de
Ca (ICa) habitualmente por debajo de las cifras recomendadas, en funci¨®n de la edad gestacional
para determinar: 1) si los cambios se asocian a la ICa; 2) si podr¨ªan prevenirse por suplementaci¨®n
c¨¢lcica y 3) si depender¨ªa de la fuente de Ca utilizada. Se dividieron en tres grupos: G1 sin
suplementaci¨®n, G2 y G3 recibieron 500 mg/d de Ca como leche o citrato, respectivamente. Al
final la ICa permaneci¨® por debajo de la cifra adecuada (IA) en el 50% de las mujeres. La
1¦Á,25dihidroxivitamina D aument¨® desde el 1er. trimestre (T); la fosfatasa alcalina ¨®sea lo hizo al
final del 2do.T sin diferencias entre los grupos; el telop¨¦ptido carboxilo terminal del col¨¢geno tipo
I (¦Â.CTX: marcador de resorci¨®n) aument¨® desde fines del 1er.T. con niveles superiores al final
del 3er.T en las que no recibieron suplemento. El ¦Â.CTX vari¨® inversamente con la ICa,
presentando aumentos de hasta cuatro veces con ICa por debajo del 50% de IA. Por lo ello, la
respuesta esquel¨¦tica durante el embarazo no ser¨ªa totalmente independiente de la ICa,
especialmente cuando es muy baja, produciendo aumento del remodelamiento ¨®seo ya que,
posiblemente, la mayor absorci¨®n intestinal no alcance a cubrir las necesidades del feto en
desarrollo. Este hecho se traducir¨ªa en una resorci¨®n mayor sobre la formaci¨®n, induciendo
p¨¦rdida de hueso trabecular (metabolicamente m¨¢s activo). El consumo adecuado de Ca,
especialmente l¨¢cteos, limitar¨ªa dicho aumento beneficiando en especial a mujeres j¨®venes que
a¨²n no han alcanzado el pico de masa ¨®sea o a mult¨ªparas con escaso per¨ªodo de tiempo entre un
embarazo y el siguiente.
ABSTRACT:
This longitudinal study evaluated changes in bone turnover in 150 healthy pregnant women
(Obstetrics Section, Hospital D. Paroissien, La Matanza) with habitual low dietary calcium intake
(CaI) as a function of the gestational age to determine: 1) the association between bone turnover and
CaI; 2) the effect of different sources of calcium supplementation on bone remodeling during
pregnancy. Women were divided in three groups: G1: without supplementation, G2 and G3 received
500 mg/d of Ca as milk or citrate, respectively. Bone remodeling throughout the gestational period
was assessed by bone specific alkaline phosphatase (b-AL) and serum carboxy-terminal Ctelopeptide
cross-linked of type I collagen (¦Â.CTX). Results: The 1¦Á.25hydroxyvitamin D levels
increase significantly from the 1st. trimester (T), the b-ALP from the 2nd T without differences among
the groups. the ¦Â.CTX increased during the 1st.T and women receiving no supplementation exhibited
the highest increment during the 3rd.T. The increment in serum ¦Â.CTX correlated negatively with
dietary CaI suggesting greater bone turnover at low intakes. Conclusion: These results suggest that
skeletal response to pregnancy may not be entirely independent of maternal calcium intake,
especially in women with customarily low calcium intake, and could induce an increment in bone
turnover that may affect bone mass status. Mothers, specially young or multipararous pregnant
women with a short period of time between one and the following pregnancy, may benefit from
higher CaI, specially from milk products.
