Objective. To analyse how the epidemiological profile modifies systemic involvement measured at the time of diagnosis in a large international cohort of patients diagnosed with primary Sjögren syndrome (SS).Patients. The Big Data Sjögren Project is an international, multicentre registry formed in 2014 to take a ?high-definition? picture of the main features of primary SS at diagnosis by merging international SS databases. By October 2017, the database included 9545 consecutive patients recruited from 22countries of the five continents.Results. Baseline ESSDAI score was available in 9118 patients (93% female, mean age at diagnosis 53 years). The median ESSDAI score at diagnosis of the entire cohort was 4 (IQR 1-8).
