RIGHT LOBE SPLIT LIVER TRANSPLANTATION COMPARED TO WHOLELIVER TRANSPLANTATION, LESSONS LEARNED AT A SINGLE CENTER EXPERIENCE

Introduction: Split liver transplantation was proposed as a novel alternative to increase the liver donor pool as well as transplant applicability. Split liver transplantation has become an accepted alternative for pediatric recipients, but its use for adult recipients remains controversial, in spite of the progressive gap between adult patients waiting for liver transplant under the MELD systemand the current number of cadaveric donors. We aim to present our experience using Right Extended Split Grafts (RESG) and compare them to match whole graft recipients (WL).Material & Methods: A single center retrospective review of RESG was performed between 8/2009 and 1/2015. Fifteen RESG were implanted, 13 in adult recipients (25 to 68 yrs old) and 2 in pediatrics (13 and 15 yrs old); 80% of the RESG were performed ex-vivo. In the WL group 29 adult recipients (21 to 67 years old) received organs procured from brain death donors. Recipients of left liver transplants, multi-organ transplants, re-transplants and living-related organ donor were excluded. We investigated recipient and donor demographics, recipient outcomes, patient and graft survival and surgical complications.Results: DRI for the RESG was 1.9 vs. 1.3 for the WL group (p
