Experiences with primary care for children in the Catalan population.

Objective. Spain established a new model of Primary Care (PC) in 1986. The 2006 Catalan Health Interview Survey (HIS) evaluated the population perspective about PC for children by health and socio-demographic characteristics.Cross-sectional study of a representative sample of the population aged 0 to 14 (n=2,200). Parents informed their experiences with PC through 23-items selected from the Primary Care Assessment Tool. Independent variables were the child’s age, sex, perceived health, chronic conditions, psychiatric disorders (Strengths and Difficulties Questionnaire), number of people in the household, parent’s country of born, parents educational level, social class, and child’s healthcare coverage. Multivariate models were fitted to estimate odds ratios (OR) and their 95% confidence intervals (CI) to analyze the relationship between scoring low in six characteristics of the PC with health and socio-demographic variables.First contact was more likely to be scored low in families living in overcrowded households (OR=1.65;CI=1.00-2.71), and by foreign-born parents (OR=1.78;CI=1.05-3.01); longitudinality, when children had psychiatric disorders (OR=1.75;CI=1.35-2.27); comprehensiveness, when children had psychiatric disorders (OR=2.05;CI=1.22-3.45 in available services) and by foreign-born parents (OR=2.04;CI=1.55-2.69 in received services); and cultural competence, when parents were foreign-born (OR=1.82;CI=1.23-2.71) or their occupation corresponded to a disadvantaged social class (OR=1.95;CI=1.29-2.96). First contact, longitudinality and comprehensiveness were less likely to be scored low when children had double healthcare coverage. There were not variables associated with experiences with coordination.Several attributes of PC are more likely to be experienced worst when children had mental health disorders, and parents are foreign-born. Further analysis will include the characteristics of PC sources and services visited.
