BACKGROUND:OnehospitalinthecityofCali,Colombia,oftheECLAMC(Latin-AmericanCollaborativeStudyofCongenitalMalformations)network,reportedtheunusualoccurrenceoffourcasesofsirenomeliawithina55-dayperiod.METHODS:AnECLAMCroutineforclusterevaluation(RUMOR)wasfollowedthatincluded:calculationsofobserved/expectedratios,sitevisits,comparisonwithcomprehensivelycollectedlocal,SouthAmerican,andworldwidedata,clusteranalysis,andsearchforriskfactors.RESULTS:AllfourCalisirenomeliacaseswereborntomotherslivingina2km2area,inneighboringcommunes,withinthemunicipalityofCali.ConsideringthetotalbirthsofthecityofCaliasthedenominator,andbasedonECLAMCbaselinebirthprevalencerates(per100,000)forsirenomelia(2.25,95%CI:2.66,3.80),theclusterforthiscongenitalabnormalitywasunlikelytohaveoccurredbychance(observed/expectedratio55.77;95%CI:1.57–14.78;p5.002).Noconsistentcommonfactorwasidentified,butvicinitytoanopenlandfillasthecausecouldnotberejected.AnotherECLAMChospitalinSanJusto,BuenosAires,Argentina,reportedthreefurthercasesbutthesedidnotseemtoconstituteanonrandomcluster.CONCLUSIONS:Themethodol-ogyusedtoevaluatethetwopossibleclustersofsirenomeliadeterminedthattheCalisirenomeliaclusterwasunlikelytohaveoccurredbychancewhereasthesirenomeliaclusterfromSanJustoseemedtoberandom.BirthDefectsResearch(PartA)85:112–118,2009.2008Wiley-Liss,Inc.
