Doppler ultrasonographic evaluation of bitches suffering from cystic endometrial hyperplasia ? pyometra complex under medical treatment

Uterine ultrasound is one of the most important diagnostic and monitoring tool in canine cystic endometrial hyperplasia ? pyometra complex (CEH-P). Bitches suffering from CEH-P show increased uterine blood flow when compared to bitches with non-infectious uterine conditions (1). The aim of this study was to evaluate uterine blood flow changes in medically treated bitches suffering from CEH-P. Seven, 8 to 14 years old, diestrous (15?60 days after the end of the previous estrus) bitches suffering from CEH-P (open: n=5 and closed cervix: n=2; ultrasonographically diagnosed) were included in this study (2). The bitches were treated once a day with cabergoline 5 μg/kg PO for 7 days and cloprostenol 1 μg/kg SC for 14 days (3). Amoxicillin-clavulanate at 12.5 mg/kg SC was administrated throughout the treatment period. The animals were clinically and ultrasonographically evaluated before the beginning of the treatment (day 0) and on days 3, 7, 14, 21 and 28. The widest cross-sectional diameter (D) and the internal diameter (ID) of uterine horns were measured. Color Doppler was conducted to localize uterine arteries at both sides of the uterine body, and pulsed-wave Doppler was performed to obtain the waveforms. Peak systolic velocity (PSV), end diastolic velocity (EDV) and resistance index (RI) were calculated for each waveform. D, ID, PSV, EDV and RI were analyzed by repeated measures ANOVA followed by Tukey test. On day 3, 5/7 animals were afebrile and had normal appetite. One closed (1/2) and one open (1/5) cervix bitches that did not improve either ultrasonagraphically or clinically, on days 3 and 7, respectively were ovariohysterectomized and excluded from the statistical analysis. The other closed CEH-P female (1/2) opened its cervix 36 hours after initiation of treatment. In all the bitches (5/5), D and ID decreased throughout the study (P
