Objective: NeurocognitivedysfunctionisconsideredasthemainpredictorofoveralloutcomeofBD. The issueofwhetherneurocognitivedysfunctioninBDisprogressive?or not?has becomecriticalinthe effort todefine stagingmodelsforthesedisorders.Dataaboutcognitivedysfunctionevolutionarescarce and contradictory.Whilesomestudiesshowedaprogressivepatternothershavefoundastableformof evolution. Methods: TwentyfourpatientswithBDaged60yearsorolder(E-BD),24patientswithBDaged40years or younger(Y-BD)and20healthycontrolsmatchedbytheE-BDgroupwereevaluatedwithtraditional clinical instrumentsandanextensiveneuropsychologicalbatterywascompleted.WeusedANOVAand Chi-squaredforcomparisons.RawscoreofneurocognitivetaskswastransformedtostandardizedZ-score from thenormativedataofeachtesttoavoidtheeffectofage.InordertodecreasetheriskoftypeI errors, one-waymultivariateanalysisofvariancewasconducted. Results: Despite havinganillnessdurationthatwas4timeslonger,E-BDdidnotdifferintermsofkey cognitivedomainscomparedtoY-BD.Thesedatadonotsupportthehypothesisofaprogressionof cognitivedysfunctionduetoillnesschronicity.
