Analysis of electrical current distribution in the thorax during radiofrequency cardiac ablation: preliminary results from a 2D computer model

Almost all radiofrequency cardiac ablation computer models simplify reality by limiting the computational domain to a fragment of tissues around the active electrode. However, since constant power is currently employed, it is crucial to know how much power is absorbed at the target site and how much by the surrounding tissue. We built a 2D model based on a cross-sectional medical image including many features of thorax tissues. The 2D results indicated the tissue structures that alter the current distribution and hence should be included in future 3D models. In particular, the results suggest that these models should be based on a truncated thorax and incorporate the following tissues: thick subcutaneous layer at the side corresponding to the dispersive electrode, spinal column, lungs, aorta, and a sphere around the active electrode mimicking the cardiac chamber. Even though the lungs did not significantly distort electrical current distribution, their effect is possibly important due to their large volume and proximity to the active electrode.
