Sex and age related differences in allostatic load index in patients with general anxiety disorder treated with alprazolam

IntroductionTo ensure a favorable assessment of the cumulative influence of stress on health, Seeman and Crimmins first developed Allostatic Load (AL)index in 1997 and proposed indicators for the functioning of the main potentially stress-affected systems [1],[2]. Sex differences in stress response and stress-related diseases susceptibility, have been described in the literature with controversial results; women are less prone to cardiovascular disease and mortality than men before menopause, but also women presented higher levels of inflammatory markers [3],[4]. Experimental studies demonstrated sex differences in stress response of neural remodelling pattern with more intense compromise in females, nevertheless, the empirical evidence is small, especially among special populations with psychiatric disorders [5]. In this observational study we determined the effects of sex and age on AL index, in a cohort of patients with general anxiety disorders treated with alprazolam during 12 weeks, before and after treatment.MethodsAll patients included in this study met criteria for general anxiety disorder codified in Axis I of DSM IV (American Psychiatric Association). In all patients the HAM-A (Hamilton anxiety scale) was determined, (>6 points indicated anxiety). For measure AL index, 18 clinical and biochemical parameters were determined at 10 ±1 AM. One point was added for each abnormal parameter according to Crimmins and Seeman modified criteria (Crimmins et. al. 2003). AL index determination was before (- 1 week) and after 12 week of treatment with alprazolam (0.25-1mg/t.i.d). AL parameters comprised cardiovascular, metabolism and inflammatory variables. Univariate analysis (Tow-way ANOVA), Student´s t-test (related variables) and Pearson correlations were determined.ResultsFifty-four patients, 35 females (48.6 11.7 years) and 19 males (44.2 12.8 years) with general anxiety disorder were included; 28 patients with
