DEMENCIA: MIENTRAS EL PENSAMIENTO Y LA MOTIVACIÓN SE
CONSERVEN LA INTEGRIDAD
DE LA
MENTE ESTARÁ A SALVO A mayor presencia de trastornos cognitivos
y conductuales, en cantidad como en calidad, mayor probabilidad de demencia.
Aunque las adquisiciones más tardías en el desarrollo humano suelen verse como
sinónimo de funciones cognitivas de mayor nivel y calidad, esto no es del todo
cierto. Las funciones cognitivas más elevadas, no sólo implican discriminación,
memoria, praxias, gnosias multimodales y razonamiento, sino también una
interacción con las motivaciones y el medio ambiente. Si una persona no tiene
la capacidad de discriminar (consciente o inconscientemente) lo que es
relevante para su supervivencia, involucrando sentimientos, emociones y
motivaciones (e incluyendo estructuras como el hipocampo y el sistema límbico,
entre otras), esa persona no será capaz de atender y aprender adecuadamente.
Para generar pensamientos coherentes (verbales y no verbales) se necesita más
la integridad del "sistema" cognitivo que la de sus
"componentes". Si las primeras lesiones afectan al funcionamiento
cognitivo más elevado y/o su interacción con las motivaciones, el pronóstico
será desalentador. Aunque la cantidad de componentes afectados también es
importante, muchas personas con múltiples afecciones físicas y cognitivas se
adaptan perfectamente porque saben cómo utilizar los recursos preservados y cuentan
con la motivación y la capacidad de aprender nuevas estrategias para enfrentar
sus deficiencias. Apuntando a la rehabilitación y prevención, sería mejor
prestar más atención a la capacidad para resolver problemas e identificar
recursos alternativos y conservados, que insistir sobre los componentes
cognitivos que ya se han deteriorado. Cuando algunos componentes conductuales
(como la depresión) están involucrados, la farmacoterapia cuidadosa también
puede ser preventiva. DEMENTIA
HYPOTHESIS: AS LONG AS THOUGHT AND MOTIVATION ARE PRESERVED, MIND IS PRESERVED
It is well-known that the greater the presence of cognitive and
behavioral impairments, both in quantity and quality, the greater the
likelihood of developing dementia. Later acquisitions in human development may
be seen as synonymous with higher cognitive functions and higher cognitive
quality. But this is not entirely true. High-level cognitive functions not only
involve discrimination, memory, praxias, multimodal gnosias and reasoning but
also their interaction with motivations and the environment. I.e., if a person
does not have the ability to discriminate (consciously or unconsciously) what
is relevant for survival, which is very much dependent of feelings, emotions
and motivations (involving the hippocampus and the limbic system among other
structures) such person won?t be able to attend and learn adequately. Besides,
and in order to get coherent thoughts (verbal or non verbal), the integration
of the high-level cognitive "system"-rather than the integration of
their "components"- needs to be preserved. If the first brain lesions
affect the highest level of cognitive functioning, and/or their interaction
with motivations, the prognosis will be discouraging. Although the quantity of
components is also important, many persons with multiple physical and/or
cognitive impairments (including, for example, persons with specific agnosias,
apraxias or amnesias) are perfectly adapted because they know how to use their
resources or have the motivation and the ability to learn new strategies so as
to cope with their impairments. So, for rehabilitation and prevention it would
be better to pay more attention to the ability to solve problems and identify
alternative and preserved resources rather than insisting upon cognitive
components that have already been impaired. However, when some behavioral
components (such as depression) are involved, careful pharmacotherapy may as
well be preventive.
