DOSIS (Dosimetry Optimization System and Integrated Software): An integrated toolkit for patient specific internal dosimetry

Objectives: To provide a tool for planar (2D) and 3D dosimetry at organ and voxel-level, as well as to develop a dosimetry platform based on full stochastic methods for alpha, beta, and gamma emitters used inradiopharmaceutical applications.Methods: DOSIS is based on Boltzman radiation transport equation. Photon-energy and charged-particle deposition are derived. Procedures for 2D and 3D dosimetry (organ and voxel level) follow those illustrated in MIRD pamphlets 16 and 17, but based on deterministic Fourier transform kernel convolution. Moreover, full stochastic Monte Carlo techniques (FSMC) are used (PENELOPE and FLUKA codes). The software toolkit has user friendly interactive graphic interfaces. Metabolic and anatomic images, and the dose map results are handled by dedicated packages. Generalized gamma functions (1) are implemented for 3D distribution comparisons.Results: DOSIS was preliminary validated in clinical cases against standard procedures (2) used for dosimetry in peptide receptor radionuclide therapy, showing good agreement and user friendliness. Good agreement was also found comparing the S voxel approach vs. the full stochastic one (2-3). The application of PENELOPE or FLUKA codes for FSMC to homogeneous and heterogeneous phantoms showed results with negligible differences.Conclusions: This study demonstrated the DOSIS reliability. The toolkit provided promising results, in agreement with standard methods used in clinics.
