Resumen:
Recent onset psoriatic arthritis induces earlyproatherogenic inflammatory signsR. Kolliker1,2, A. Ares1, G. Melito1, F. Capani1,2 2Laboratory of Cytoarchitecture andNeuronal Injury, Institute of Cardiologic Research (ININCA),University of Buenos Aires-CONICET, Buenos Aires, ArgentinaArterial immune mediated inflammation participates centrally inall stages of the development of atherosclerosis, from the initiallesion to the end-stage thrombotic complications. Althoughemerging evidence supports augmented cardiovascular morbidityand mortality in cutaneous psoriasis (PsC) and psoriatic arthritis(PsA) as compared to the general population its underlyingmechanism is poorly understood. Here we analyzed the inflammatoryburden in recent onset of PsA patients without traditionalcardiovascular risk factors (CVRF) in a transversal studymeasuring carotid intima media thickness (cIMT) (measured withecodoppler), and proatherogenic inflammatory molecular markerslike C-reactive protein (CRP), Interleukin 6 (IL-6), and solubleintercellular adhesion molecule-1 (sICAM-1) in comparison withcontrol patients. cIMT values are similar in PsA (0, 59 _ 0.045*)and PsC (0, 62 _ 0.10) patients. However, both of them were significantincrease compared with control (0.436 _ 0, 05). Regardinginflammatory markers IL-6 serum levels in patients with APswas higher than PCs (18 _ 2.9) and healthy controls (16,3 _ 2.5) but the difference did not achieve statistical significance(*P > 0.05). On other hand mean of sICAM-1, value frompatients with recent onset of PsA is significant higher than controls.PsC remain without significant changes compared to control(*P > 0.05). In addition mean value from patients withrecent onset of PsA is significantly higher than in controls(*P < 0.05) and PsC group. Overall, preliminary findings suggestfor the first time that patients with early PsA, without evidenttraditional CVRF have significant increased values of cIMT,sICAM-1 CRP against the general population control group. This data strongly supports that early CV molecular markers areincreased after the first symptoms and signs of this disease evenin the absence of traditional cardiovascular risk factors.
