Zhou et al. published an elegant paper on the risk of coronary heart disease (CHD) in hypertensive patients with high circulating angiotensin II (Ang II) levels [1].The paper [1] has 3 key findings. First, that coexistent hypertension and high circulating Ang II level may be a predictor of CHD incidence. Second, the study was carried out over a 10 year period in an inner-Mongolian minority population in China, accounting for nearly 3500 individuals. Third, the 3.17 million Mongolian inhabitants are a quantitatively relevant medical target. Moreover, data were collected under strict methodological conditions, hardly losing information over the follow-up period.Hypertensives with a circulating Ang II blood level >49 pg/mL had the highest risk of CHD of all 4 subgroups under study, namely, normotensives with Ang II ≤49 pg/mL, normotensives with Ang II >49pg/mL, hypertensives with Ang II ≤49 pg/mL, and hypertensives with Ang II >49 pg/mL. They actually had a 3-fold increased risk compared with normotensives with Ang II blood level
