Patterns of necrosis and survival in myocardial infarction: natural history versus contemporary treatment

Resumen:
We sought to compare patterns of ST-segment elevation myocardial infarction (STE-MI) before the introduction of reperfusion therapies, and with contemporaneous sta-te-of-the-art treatment of STEMI. Two cohorts of patients admitted for STEMI and without previous myocardial infarction constituted the study population. Group A (natural history) comprised a consecutive series of autopsies performed in patients dying of STEMI admitted between the years 1975 and 1977. Group B comprised a contemporary consecutive cohort of patients who underwent primary percuta-neous coronary intervention (PCI) for STEMI and were examined with delayed-en-hancement computed tomography immediately after completion of PCI. Demogra-phical characteristics overall did not differ between groups. All patients in group A showed evidence of necrosis, whereas only 14 (47 %) in group B showed evidence of irreversible myocardial damage.
