End of apposition period in bone mass peak and bone remodeling markers

Adequate bone apposition is vital for optimal skeleton growth. Thus, adequate reference values of biochemical bone markers are critical. Objective: To determine the end of bone apposition by biochemical markers and establish reference values in healthy subjects, divided by decades between 20 and 49 years of age. [/Population/]: 146 premenopausal women and men without interventions affecting bone and mineral metabolism. Excluded: subjects with serum calcium, serum phosphorus, or creatinine out of normal range (n = 20). [/Methods/]: Fasting serum (8–10:30 am) evaluated for C-terminal cross-linking telopeptide of type I collagen (CTX) and osteocalcin (BGP) by Elecsys Roche (intra and inter-assay CV were 1.6 and 6.1%, and CV 6.9 and 2.9%, respectively) and bone alkaline phosphate (FAO) by precipitation (intra and inter-assay CV were 1.3 and 8.0 %, respectively). Results: 126 subjects (61 men + 65 women) divided by decades (20–29; 30–39; 40–49) depending on biochemical parameters. Data was analyzed by decade, and we observed that BGP and CTX was higher bet. 20–29 than between 30–39 (P < 0.001) and 40–49 (P < 0.001; P < 0.01), respectively. Men did not show significant differences with women between 20 and 29. Among 30–39 men, they showed higher FAO than women with no difference in CTX and BGP. Finally, men between 40 and 49 had higher CTX, FAO and BGP. In both sexes, a positive correlation was observed between CTX and BGP (r = 0.6, P < 0.001), BGP and FAO (r = 0.35, P < 0.001), CTX and FAO (r = −0.3, P < 0.02). Conclusions: Bone remodeling speed decreases in the 3rd decade. The difference observed in BGP and FAO levels tallies with the different phases of osteoblastic development that both represent.
