Levels of 25hydroxyvitamin D (25HOD3), Parathormone (PTH) and bone alkaline phosphatase (bAL) in institutionalized women over 65 from Lleida city (Spain) and suburban Buenos Aires (Argentina).

Vitamin D insufficiency is common in institutionalized elderly people. Argentina has a high prevalence of vitamin D insufficiency but in Spain it had not considered a common problem because of the high level of sun irradiation. Also, these two countries are traditionally different in terms of nutritional habits (red meat vs. fish). The objective of the present work was to compare vitamin D status, levels of PTH and bone bAL in women over 65, living in institutions with similar social status in cities with a similar solar irradiation index: Lleida (Spain) (43 0NL) and a suburban Buenos Aires (BA) area (340SL). During the end of the summer 81 women (50 from Lleida aged 82±7 and 31 from BA aged 81±8) were studied. None of them received vitamin D supplements or any medication that could affect bone metabolism. A dietary survey was recorded (calcium and vitamin D consumption) and fasting serum was obtained. Results (mean±SD) in Lleida vs. BA, respectively were as follows: Hemoglobin (g/dl): 12.6±2.4 vs 13.1±1.4; albumin (g/dl): 3.73±0.11 vs 3.51±0.35; 25HOD3 (ng/ml) (RIA, Diasorin): 10.9±5.8 vs 12.4±5.4. A total of 50% in Lleida and 43% in BA had 25OHD3 lower than 10 ng/ml; mmPTH (pg/ml)(RIA) were 100.0±112.7vs. 119.6±150.6. A total of 24% in Lleida and 20% in BA had PTH over 100 ng/ml. The levels of 25HOD3 inversely correlated with PTH in the two cities (Lleida: r=0.33, p<0.035 and BA: r=0.40, p<0.046). The bAL (IU/L)(wheat germ precipitation-colorimetric) were: 55±9 vs. 65±20 (p<0.01), respectively. Sun exposition was low in both groups. LLeida had greater fish consumption (70 vs. 22 g/day) and lower red meat consumption was than BA. Dairy product consumption was similar (calcium intake=800 mg/d), Vitamin D intake was lower in Lleida (1.7 vs. 4.0ug/d) where dairy products are not enriched. Conclusions: Vitamin D deficiency was important in both population groups.
