Endometriosis, a chronic disease characterized by endometrial tissue outside the uterine cavity, affects 1/4 of young women and is associated with chronic pelvic pain and infertility. An in-depth comprehension of the pathophysiology and effective treatment strategies of endometriosis are largely elusive. Inadequate immune surveillance is significantly involved in the pathophysiology of endometriosis, and key findings are summarized in the present review. We further discuss the role of adhesion molecules, carbohydrate-protein interactions, angiogenesis and introduce a concept of endometrial dissemination as result of a neuro-endocrine-immune disequilibrium in response to high levels of perceived stress caused by cardinal clinical symptoms of endometriosis.
