Pattern of care in people with type 1 diabetes in the International Diabetes Management Practices Study (IDMPS)

The IDMPS is being performed to gather data that reflect current practices in the care of people with diabetes in different areas of the world. This multinational, cross-sectional, observational study is collecting data in 1-year cycles for 5 years. Type 1 diabetic patients (¡Ã18 years) seen in current medical practice by physicians managing diabetic patients were enrolled during a 2-week recruitment period. In addition to monitoring HbA¡ø1c and fasting blood glucose (FBG), information about insulin self-injection, self-titration, diabetes education and frequency of visits to their physician was collected. Data are presented from 1327 people (51% men) with type 1 diabetes in 11 countries (Eastern Europe, Asia, Latin America). 76% of people had HbA¡ø1c tested with an average of 4 tests in the last 2 years. Mean HbA¡ø1c was 8.2¡¾1.96% and 27% of people had an HbA¡ø1c <7%. Laboratory testing for FBG was done on average 9 times/year. Mean FBG (laboratory) was 161¡¾75 mg/dL. 73% of people self-monitored their blood glucose (SMBG): FBG was being monitored by SMBG 3 times/week and post-prandial glucose 10 times/month. Self-injection of insulin was reported in 97% of people and self-titration in 69%. 39% were considered to be ¡°at target¡± according to the physician, and among these people the mean HbA¡ø1c was 6.9¡¾1.49%. Patients were visiting their physicians on average of 7 times/year. Training was provided by diabetes educators to 84% of patients. Our data show that people with type 1 diabetes are poorly controlled (in terms of HbA¡ø1c and FBG) although the majority is being educated and monitored regularly for their condition. This suggests that improved education of patients and their providers is necessary to get a more interactive and effective care of people with type 1 diabetes.
