Study of the T-wave spectral variance based on electrical remodeling after myocardial infarction

Resumen:
Experimental and clinical studies have shown thatbeat-to-beat variability of ventricular repolarization morphology, which can bemeasured by T-wave spectral variance (TSV) index, is associated withventricular tachyarrhythmias. In the present study we tested TSV index duringhealing phase and healed infarct phase using 12 standard ECG. The ECGs of healthysubjects (n=48) and the ECGs of patient with anterior myocardial infarction(n=21), one within the first seven days (MI7) and the other recording after 60days (MI60) of myocardial infarction, were obtained. Results indicate that TSVindex showed significant differences between MI7 (healing phase of myocardialinfarction) and control, in all ECG leads, excepts for II and V1 leads. Also,TSV in MI60 (healed phase of myocardial infarction) with respect to controlsituation showed statistical significance differences in I, II, aVR and inV3-V6 precordial leads. The TSV results observed are concordant withelectrophysiological changes showed in several experimental models in the electricalremodeling process. Finally, we conclude that TSV index offers a robust toolfor evaluating beat-to-beat repolarization variability during different stagesof myocardial infarction.
