A diet enriched in extra virgin olive oil prevents placental morphological alterations and reduces insulin resistance and circulating matrix metalloproteinase 9 in women with Gestational Diabetes Mellitus

Resumen:
Introduction: Gestational diabetes mellitus (GDM) is a prevalent disease that increases the risks of maternal, fetal and placental complications and leads to the programming of metabolic diseases. Changes in placental morphology are associated to an adverse programming. Previously, we found that maternal supplementation with extra virgin olive oil (EVOO) reduced matrix metalloproteinases (MMPs, markers of the pro-inflammatory state if overproduced) in the placenta of GDM patients1. The ratio between triglycerides (TG) and HDL-Cholesterol as well as fatty acid binding protein 4 (FABP4) in blood are considered markers of insulin resistance. In this work, we hypothesized that a maternal diet enriched in EVOO modulates placental morphology, as well as TG/HDL-Cholesterol ratio, FABP4 levels and MMP9 activity in blood from patients with GDM.Methods: Fifty healthy (Control) and GDM patients were enrolled (protocol approved by Hospital Pirovano Ethics Committee), and all of them were advised to follow the WHO diet for pregnancy. GDM patients were randomized to receive or not three tablespoons/day of EVOO from week 24-28 of gestation until delivery. At term, maternal blood was obtained to evaluate FABP4 levels (WB), TG and HDL-Cholesterol levels and MMPs activity (zymography). After delivery, placentas were morphologically evaluated.Results: The placentas of women with GDM showed no changes neither in diameter nor perimeter, but were significantly thicker when compared to Controls (15%, p
