Resumen:
Trypanosoma cruzi infection may be acquired through the triatomid insect vector, blood transfusion and the transplacental route leading to Congenital Chagas disease.  In recent years, the success in the control of vector-transmitted Chagas’ disease and screening programs in blood banks has uncovered the public health importance of congenital T.cruzi infection. Family clustering and second-generation of congenital infection in vector-free and non endemic areas show emergent modalities of transmission leading to the urbanization of Chagas disease.  Epidemiological, Clinical and immunological features of Congenital infection are discussed. Subsequently, we consider risk factors in humans and in experimental model that may contribute to our understanding of the mechanisms of congenital transmission. Evidences on the role of maternal parasitemia and differential histotropism of T.cruzi populations in placental infection and congenital transmission are also analysed.  The main procedures for parasitological, molecular and serological diagnosis, as well as trypanomicidal treatment and follow-up are described.  Finally, international consensus recommendations of strategies for control, diagnosis and treatment of congenital Chagas disease are detailed
